Steps to Becoming a Proxy

If you are new to picking up food for someone else, either an individual or a family, you will need to help them complete two required forms (see below) and the Proxy form is required on your first visit to the Food Closet. You must also bring picture identification of yourself as proxy and also a picture ID* of the person you are picking up food for on that day.

This form is required on your first visit. It authorizes you, as proxy to pick up on behalf of client and the form must include name/address and D.O.B. of client.

Proxy can assist client in completing this form. This form registers the client into an online platform which allows them access to food distribution throughout Sacramento region.

*If no “official” ID is available, we’ll require a piece of mail that contains the name and address to establish the client record.

Proxy Requirements

Once you have been authorized by the client to pick up on their behalf, you can visit the Food Closet once a week to pick up their full food assistance. To facilitate the check-in process on each visit, the proxy must present picture ID of the client or the blue Food Bank card with barcode. The picture ID can be an image on a smartphone.

· To pick up for more than one client, guests must return on another distribution day.

· Proxy cannot make changes on client’s registration profile without client being present.
